We have new office policies in effect.

Appointments

Patients are generally seen in our offices by appointment. We make all attempts to accommodate appointments with the provider of your choice as quickly as possible.

Please arrive at least 5 minutes prior to your scheduled visit time, 20 minutes for new patients and appointments for annual physicals.

Patients need to be on time for their appointments. If a patient is more than 10 minutes late for an appointment, the appointment may need to be rescheduled.
Insurance and Payments

Our providers participate with Medicare and most insurance plans.

For your convenience, we will file your primary insurance claims if you provide us with the correct billing information.

All patients must have a secure method of payment on file that guarantees payment for services rendered. Any time a payment method does not satisfy a charge and a statement has to be generated there will be a $5 statement fee assessed.

$10.00 fee for copays not paid at time of service.

We collect $100.00 up front for patients who have not met their annual deductible.
Phone Calls

Our providers see scheduled patients all through the day and usually cannot be called away to talk to patients on the phone.

The medical assistants are trained to handle your phone questions initially, and will provide advice, offer an appointment, or consult a physician as needed.

If you still need to speak with a provider, they will recommend that you come in for an appointment or schedule a telemed visit.
Medication Refills

All regular medications, non-controlled/non-narcotic prescriptions, require a follow up appointment with your provider every 3-6 months (more frequent if necessary). This will be considered a regular office visit.

No prescriptions will be refilled on Friday, Saturday, Sunday or Holidays.

Patients must have been seen in the last 6 months before a refill is given.

All prescription refill requests should originate from the patient by contacting their pharmacist asking to request the refill electronically.

All prescriptions for controlled substances require a face-to-face encounter (with RARE exceptions, telemed may be available) in the office with only your treating doctor. It is our strict policy that there will be no refills of controlled substances called-in over the phone or by an after-hours physician when the office is closed. NO EXCEPTIONS.

Medication Prior Authorizations can take up to 14 business days to complete.
